    Workers’ Compensation

    Workers’ Compensation provides benefits and assistance to any employee who is injured or serious illness or the job. The New York City Law Department/Workers’ Compensation Division serves as the insurance carrier for CUNY. All employees are eligible for Workers’ Compensation.     • DP2002 Form – Employee Election – Please note that an election must be made on the DP2002, in the event of a medical leave. Failure to make an election will automatically place you in option 2, which will be an unpaid leave status.
    • Employee Notice of Injury
    • Supervisor Report of Injury
    • Witness Statement
    • Workers’ Compensation Insurance Coverage

    The above forms, required by the New York State Worker’s Compensation Board (WCB), should be completed and promptly returned to the OHR. Upon receipt, the OHR will forward the forms to the New York City Law Department – Worker’s Compensation Division, wherein a decision will be rendered as to the compensability of the claim. The Workers Compensation Division processes all claims of injuries and accidents.

    It is imperative that in the event of an injury or an accident, immediately contact the Public Safety Office and notify their supervisor. In addition, the employee must notify Human Resources within 10 days of injury.
